Picture & Picture
Example: (I added a border to the icon, so you can it better)
Step 1: Jasc Paint Shop Pro
[used the image that you want to use -or- you can used mine image.]

Step 2: Crop it (96 x 96)
Step 3: Duplicate the layer (3x)
--1st layer set on soft light
--2nd layer set on hard light
--3rd layer set on burn
Step 4: New Layers (#8080FF)
--set screen
--Opacity:50%
Step5: New Layer (#FF4040)
--set burn
--Opacity:28%
Step 6: New Layer (#50506A)
--set soft light
--layers>fatten
Step 7: Crop & Border
--face
--bordercolor:white
--example:
(I added a border to the pic, so you can it better)
--Pic over orginal face.
Step 8: New Layer
--Move this layer behind the Pic over orginal face
--Brush:color:#F0C4D3
Step 9: Border (2x)
--color: white
(I added a border to the icon, so you can it better)
Voila! Done
